Driver Safety Performance History Requests
Process of obtaining new driver’s previousdriving history from previous employers.
Responsible Department or Person
· New Hires - Drivers
· Branch Administrators
· Human Resource Department
· Shop/Loss Prevention Department
Completion Time and Frequency
· BA’s - Immediately upon completing onboarding form for new Driver.
· Shop/Loss Prevention- Immediately upon notification of new Driver fromthe BA or HR Department.
Process
· The BA will electronically file the executedSafety Performance History Records Request form into the BA’sDrivers Compliance Dropbox folder.
§ Choose Safety Performance History folder
§ Choose the branch the Driver is being hiredfor
§ File in the subfoldernamed Pending.
· The Shop/Loss Prevention department will send thecompleted Safety Performance History RecordsRequest form to the previousemployers listed on the completed DriverSafety Performance HistoryRequest form via emailor mail.
· After the previous employer responds, the Shop/LossPrevention department will file the completedform in the BA’s DriversCompliance Dropbox folder

§ Choose the branch the driver is hired for
§ File in the subfoldernamed Complete.
· After filing it into the Complete folder,the shop will notify the HumanResource department to save a copy into the employee’s permanent Employee File.
· A second attempt to retrieve the previous employer’sdriver’s safety performance history is not necessary. We will be compliance with DOT with one documented attempt to retrievethe driver’s pervious history.
