                Driver Vehicle Inspection Report

                 Inspection of all DOT trucks before leaving Russell Landscape premises for the first time each day and non-DOT                        trucks once per week.

Responsible Department or Person

·                 All drivers in all departments.

Completion Time and Frequency

·                 Every time a Russell vehicle leaves our premises for the first time each day.

·                 All non-DOT required trucks will need to be inspected once per week.

Process

·             Prior to driving a vehicle out for the first time of the day, all drivers must complete a DVIR - Driver Vehicle Inspection Report. The manager or mechanics will have a copy of the form if you do not complete the Electronic DVIR on Asset Panda.

·             Ifany items on the form need attention, advise your supervisor before leaving thepremises to ensure the vehicle is clear to leave for work.

·             If there are any items marked that need attention the supervisor will contactthe Mechanic or Fleet Manager to make any necessary decision about the vehicle.

·             Once the DVIR form is completed, the driver will turnin the form at the gateto the Dispatching Manager.

·             The Dispatching Managerwill be responsible for delivering the forms to ourmechanic or filing forms in the designated area for 30 days and informing theBranch Manager of reported items.

·             The Branch Manager will coordinate repairs, if any,with the Mechanic or Fleet Manager and will add to the mechanic’s workflowthrough Cognito for thebranch found in Asset Panda.
